If you need to return an item of equipment to us, could you please include a full description of the fault along with your contact details. If you do not have a standard equipment returns form in your organisation, please complete the following form. This can either be submitted to us electronically or printed and sent with the equipment.
If you require us to calibrate the equipment following the service then please check the box.
If you wish for us to collect the equipment from you via UPS then please check the box. There is an additional charge for this service.
Following receipt of the equipment, we will make a full assessment of the work required and e-mail you a quotation for the repair.
